At the Weiser Center for Prostate Cancer, we believe in supporting our patients in all aspects of their care. In addition to our world class medical care, we offer customized resources to assist you and your loved ones during your prostate cancer journey.

Patient Navigator

Navigating a complex medical system can be challenging. At the Weiser Center for Prostate Cancer, we offer access to a Prostate Cancer Patient Experience Navigator, who is skilled in understanding you and your loved ones’ specific needs and can help make your care journey as seamless as possible. Our navigator will provide emotional, educational, and logistical support throughout your journey.

Educational Materials

Knowledge is power. On our website, we offer educational videos and resources that you and your family can use to learn about different treatment options, what to expect, and how to take care of yourselves throughout your prostate cancer journey.

Peer Mentors

A peer mentor is a prostate cancer survivor trained through Michigan Medicine’s Office of Patient Experience. Peer mentors provide support to patients diagnosed with prostate cancer. They share their own experiences, offer practical advice on managing daily life during and after treatment, help patients connect with relevant resources, and understand the toll prostate cancer can take on families.

Cancer AnswerLine™

Cancer AnswerLine™ is a free community service for patients and families who have questions about cancer. This service is available Monday through Friday from 8 a.m. to 4:30 p.m. to help answer your questions. Call 800-865-1125.

Patient and Family Advisory Committee

Established in 2011, the Rogel Cancer Center’s Patient and Family Advisory Committee (PFAC) partners patients and families with members of the health care team to provide guidance on how to improve the patient and family experience. The committee is comprised of: patients, families, clinicians, staff, and administrators who serve as a voice for patients and family members. The Weiser Center for Prostate Cancer works together with the PFAC to design our programs with the patient and family voices in order to deliver the highest quality care possible.

Patient Family Support Services (PFSS)

Cancer affects not only the organs of the body, but the whole person and family. Our Patient Family Support Services (PFSS) is made up of highly skilled clinicians who assist in transforming the overall care experience through mutual partnership with patients, families, and additional providers. Services are offered at no cost to the patient with a clinician’s emphasis to be present, compassionate, committed, and equitable to all. Patients may utilize:

  • Art Therapy
  • Music Therapy
  • Families Facing Cancer Program
  • Spiritual Care
  • Guided Imagery

Our Patient and Family Support Services Program is one of the most comprehensive of its kind in the nation offering one-on-one interventions as well as clinician-led groups and special events in partnership with community organizations throughout the year.

Prep Forms

Once you are scheduled for your appointment, you will be provided information regarding necessary forms to complete.

These include a Health Questionnaire and a Friends and Family form. We can share these in multiple ways to best suit your needs – via the links above, your patient portal, or we can print and have available at your first appointment.

Please note that at your appointment, your specific provider may have additional forms for you to complete on-site. These include an AUA SYMPTOM SCORE (AUASS), SEXUAL HEALTH INVENTORY FOR MEN (SHIM), and Expanded Prostate Cancer Index Composite for Clinical Practice (EPIC-CP).

Completing these health forms helps our care team best prepare for your treatment needs.

Using the Patient Portal

To start, you need your own account. Don’t worry; setting it up is easy.

First, go to the MyUofMHealth Patient Portal. If you already have an account with us, enter your credentials and sign in. If you do not have an account with us, please click on the ‘New User Sign Up’ button located underneath the user sign up area. Once you have logged into your account, you will be able to access your information.

After your account is set up, you can check it any time. Just log into MyUofMHealth Patient Portal and you’ll be able to access everything you need to know about your care in one place – your portal page where you can see your doctor visits, test results, and your medicines.

If you have a question for a member of our care team, the portal is a quick and easy way to send a message. Click on the “Messages” tab at the top of your screen, then click on the “Send a message” button. For portal help, visit the FAQ page.
